Allergy & Asthma Network is conducting a pair of short surveys. We want to learn more about patient and physician experiences with telehealth using a smartphone, laptop, tablet or desktop computer.
Your telehealth experience can be before and/or after March 1, 2020.
Each survey should take no longer than 10 minutes to complete. Responses are confidential and anonymous.
Patient survey about your experiences with telehealth
This survey is open to patients or caregivers who have used telehealth and who are living with a diagnosis of:
- allergies
- asthma
- COPD
- COVID-19
- eczema
Physician/Healthcare Professional survey about your experiences with telehealth
This survey is open to physicians and healthcare professionals who:
- are 30 years of age or older
- physicians, nurse practitioners, physician assistants
- specialize in allergy, dermatology, ENT, family practice or primary care, pediatrics or pulmonology
- have used telehealth to visit with patients
This survey will be open until Dec. 20, 2020.
